Job Description

The Dr. John C. Corrigan Mental Health Center (JCCMHC) is seeking a highly motivated candidate to work in the Adult Case Management Department. Incumbents of this position will provide services to chronically mentally ill adults. Will be responsible for conducting client assessments, development and writing of Individual Service Plans (ISP) with the Clinical Team; Coordinates/assess client services, such entitlement programs, counseling, and housing. Carries a caseload as assigned by Human Service Coordinator II.


Please Note:
This position is 40 hours per week. The selected candidate will work from 8:30 a.m. to 5:00 p.m. Monday through Friday. The days off are Sunday and Saturday.

  • Interviews individuals and others and reviews medical and psychiatric records to assess individual needs, completes a Comprehensive Assessment, recommends treatment, and/or service modalities: including emotional, social, vocational, financial, health, recreational, and legal factors.
  • With individuals served, develops, and writes person centered Individual Service Plans (ISP) based on these assessments.
  • Demonstrates knowledge and sensitivity to age-related and cultural needs of individuals in development and implementation of Individual Service Plans.
  • Monitors the implementation of Individual Service Plans in partnership with the individual's other providers.
  • Refers individuals' treatment to programs and reviews and assesses treatment progress and service delivery.
  • Meets with individuals and families to coordinate service delivery and to assess the need to improve services and makes referrals to other programs and facilities as needed.
  • Provides creative engagement strategies including face-to-face meetings with persons served, family and other providers to engage and sustain involvement in attaining the persons served directed goals developed.
  • Prepares and maintains case records according to DMH regulations and MHIS business rules including progress notes and other required documentation.
  • Performs other duties such as coordinating emergency or crisis intervention services, consults with service providers and community agencies on Case Management issues, providing and/or arranging transportation for individuals, and attends and participates in individual's discharge planning.
  • Coordinates, monitors, and assesses the quality and quantity of Case Management reviews; coordinates, facilitates, and documents annual and periodic ISP reviews as required by Department regulations.
  • Assists in the development of appropriate policies and procedures through collaboration with administrators and clinical supervisors.
  • Attends all mandatory meetings and completes all annual and competency training within prescribed timeframes.
  • Meet regularly with supervisor, prepare, and actively participate in supervisory process.
  • Respond to and manage client crises including the completion of Suicide Screening as needed and according to prescribed practices.
  • Monitors the status of the health and medical needs of all individuals.
  • Makes home and community visits with clients along with transporting clients to scheduled appointments, etc.
  • Coordinates referrals and placement to DMH State run and/or vendor contracted services.
  • Follow all DMH, Area, and Site protocols.
  • Familiarization with computer and data entry.
  • Performs related duties as assigned.

Preferred Qualifications:

  • Given population served, bilingual or multilingual candidates preferred; experience working in a behavioral inpatient setting preferred.
  • Highly recommend and prefer candidate who is fluent in Portuguese and has the ability to read and write in Portuguese in order to serve caseload or predominantly Portuguese speaking persons.

Qualifications

First consideration will be given to those applicants that apply within the first 14 days.

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
