Job Description

The Department of Mental Health's Brockton Multi Service Center (BMSC) is seeking an experienced Case Management Supervisor. The successful candidate will provide direct supervision to Adult and TAY Case Managers. Serve as the Transitional Age Youth (TAY) Supervisor for the Site through collaboration with CYF and Adult Departments. Co-lead Case Management team meetings to review enrolled cases. Perform Record Reviews of Electronic and Physical charts. Contract Monitor both ACCS and Clubhouse contracts. Review data and provide support for Performance Improvement activities. Actively participate in Site Committee memberships. Attend collaborative meetings with Homeless and Care Coordination networks.

Please Note
: This position is 40 hours per week. The selected candidate will work on the first shift from 8:30 a.m. to 5:00 p.m. The days off are Saturday and Sunday.



Duties and Responsibilities (these duties and responsibilities are not all inclusive):


  • Policies and procedures are implemented by the following Area Case Management protocol and ISP regulations to ensure efficient and affective service.
  • Regular supervision and case consultation is provided to Case Management personnel including secretarial staff through weekly meetings or as needed to ensure all consumers are served appropriately.
  • Data collection and quality assurance of area programs is maintained through participation in biannual meetings to ensure the gaps in services are addressed.
  • Administratively complex cases are resolved through consultation with Site Director to ensure all consumers are served appropriately.
  • The screening and hiring of new Case Management staff is participated in through interviews and reference checks to ensure staff excellence.
  • A schedule of record keeping and reporting of all pertinent documentation is maintained through established process to ensure accountability and timely responses to requests for information.
  • Regular performance evaluations of all directly supervised employees are conducted through EPRS process to ensure staff are able to perform duties as expected level of performance.
  • Case Management and administrative responsibilities are assumed in the absence of immediate supervisor to ensure program continuity.
  • Appropriate therapeutic principles are implements in treating clients and supervising supervisees' caseloads based on common values to ensure consumers receive services in a timely, dignified and compassionate manner.
  • Proactive and preventive approach to clients presenting with high risk factors.
  • Ensures that supervisee's cases are covered in their absence through ongoing awareness of issues to ensure continuity of care.
  • Ongoing and specific inter/intra agency meetings are facilitated/attended to ensure continuity of care for clients.
  • Responsibility for out-of-area issues is assumed through Area Of Responsibility Change Process to ensure that new consumers and those who move receive timely and appropriate services.
  • Orientation and training is provided to newly hired Case Managers through a standardized curriculum to ensure standards are understood and upheld.
  • Consultation is provided to shelter through regular meetings with Shelter Specialist and staff to ensure that homeless consumers receive timely and appropriate services.
  • Attend and chair, if necessary, providers' clinical/administrative meetings.
  • Liaise and corroborate with continuing care facilities as necessary.
  • Performs other duties as requested.

Required Qualifications:

  • Ability to understand, explain and apply the provisions of the laws, rules, regulation, policies, procedures, specifications, standards and guidelines governing agency and unit activities.
  • Ability to prepare statistical reports.
  • Ability to supervise, including planning and assigning work to the nature of the job to be accomplished, the capabilities of subordinates and available resources, controlling work through periodic reviews and/or evaluations, motivating subordinates work effectively, and determining the need for disciplinary action and either recommending or initiating disciplinary action, determining subordinates training needs and arranging for such training.
  • Knowledge of the principles and practices of program development and service delivery utilizing a variety of human service personnel.
  • Ability to assess case management process and results.
  • Knowledge of the principles and methods of social casework, group work and community organization.
  • Ability to establish and maintain harmonious working relationships with others.
  • Ability to exercise discretion in handling confidential information.
  • Ability to evaluate and maintain accurate records.
  • Ability to develop and present budget information and recommendations for assigned unit.



Preferred Qualifications
:

  • Previous experience working with people with chronic mental illness and/or substance use disorder(s).
  • Bi-lingual preferred.

Qualifications

Applicants must have (A) at least four (4) years of full-time, or equivalent part-time, case management or clinical/therapeutic experience (B) any equivalent combination of the required experience and the substitutions below.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.

Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.
