The Site of Care (SOC) Operations Manager will work to improve operational processes and methods in addition to providing and implementing specific action plans to fulfill organizational goals and missions. The (SOC) Operations Manager plays a key role by leading transformation of the care that is delivered to our partners, strengthening the patient-centric collaboration between our clinical services and our provider networks, and driving continuous improvements in quality and patient experience in post acute care, decreasing the total cost of care and ultimately translating to profitable growth. This manager oversees a team who are provider-facing with accountability to build a collaborative relationship with key clinical leaders in our provider organizations, analyze provider quantitative performance and operational capabilities and to develop and execute a strategic roadmap. This role requires significant leadership skills, executive presence, and the ability to make connections across organizations, disciplines, and levels of seniority.

This role will report to our SOC Lead Director.

This role will require up to 50% travel.

What will you do?
  • Collaborate with the Partner Success, Operations leadership and Clinical Teams to ensure that operational interventions are aligned with overall strategy for client.
  • Ensure the elements of the program(s) are executing to program plans, partner contract requirements, and company commitments.
  • Deliver interventions to drive improvement across Value-based care programs.
  • Establish, nurture, and utilize partner relationships to support collaboration and successful execution of organizational and partner priorities and goals.
  • Managing Site of Care operations staff to ensure timely and qualitative communication, facilitating workshops and partner sessions and ensuring coaching and training.
  • Provide program level management of day-to-day program operations.
  • Review client data, action plans, operational plans, NSOC use, and SNF escalations
    • 1:1 meetings with staff should include a discussion of their data and opportunities/challenges
    • Client Team Meetings to review actions and upcoming meetings to share data
  • Collaborate with partners within CVS Accountable Care to:
    • Identify the solutions to overcome any inconsistencies across the market
    • Capture feedback from the marketplace and the provider community that will sharpen the Care Transition strategies going forward and elevate to Senior Leadership for continuous process improvement
    • Motivate and mentor others to probe into technical and operational details to improved process, efficiency, and results
    • Contribute to a motivated work environment by working effectively to achieve common goals
We are looking for someone with:
  • 7+ years of strong background in health care delivery methods and tools, clinical performance improvement, clinical operations, population health and care management
  • 5+ years of strong Medicare and clinical program experience including clinical program delivery and performance improvement techniques on the provider side.
  • 3+ years of leadership and coaching skills demonstrated, preference for prior management experience
  • Demonstrated expertise interpreting population data, risk stratification
  • Experience working with clinical and claims data
  • Consulting or business development experience is a plus.
  • Experience and expertise implementing service models that innovate, add value, and drive operational results in health fields.
  • Experience with Episodes of Care or Value Based Care
  • Strong critical thinking skills needed to analyze medical data and reports.
  • Experience with Health Care Organizations
  • Ability to travel up to 50%.
Education
  • Bachelor's degree required
  • Master's degree in related field preferred: MBA, MHA, MPH or other Health professional degree preferred (RN, other)
Pay Range

The typical pay range for this role is:

$75,400.00 - $165,954.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company's equity award program.
