Position Summary
The Senior Clinical Strategist will work to improve operational processes and methods in addition to providing, and implementing specific action plans to fulfill organizational goals and mission. This role will report to the Executive Director of Operations. This role is key to facilitating transformation of the care that is delivered to ACO attributed lives, strengthening the patient-centric collaboration between CVS ACO Inc. and our provider networks. The role is provider/clinic/post-acute facing and is responsible for driving to continuous improvements in quality and patient experience, decreasing the total cost of care, and translating to profitable growth. The scope will span the care continuum.

This is a remote position, eligible candidates can live anywhere in the United States. The position will require travel to inpatient, clinic, and post-acute facilities. You can expect 50-60% travel nationwide.

Responsibilities
  • Collaborate with Operations leadership and Clinical Strategist and Population Health managers to ensure that operational interventions are aligned with overall strategy.
  • Ensure the elements of the program(s) are executing to program plans, partner contract requirements, and company commitments.
  • Deliver interventions to drive improvement across Value based programs.
  • Establish, nurture, and utilize partner relationships to support collaboration and successful execution of organizational and partner priorities and goals.
  • Managing stakeholders, ensuring timely and qualitative communication, facilitating work streams and partner sessions and ensuring coaching and training.
  • Provide program level management of day-to-day program operations.
  • Analyze and report on provider quantitative performance and operational capabilities and to execute a strategic roadmap. The role will have in-depth knowledge of each of the practices assigned and consistently collaborate with all stakeholders.
  • Engage acute and post-acute partners in managing high risk patients through next site of care planning, performance network utilization,management of SNF performance metrics and continuous outcomes and financial improvement.
  • The role will also be responsible for supporting clinical performance meetings where applicable.
  • Possess an ability to review, analyze, and address gaps in service excellence, policies, and/or procedures proactively; identify issues and implement solutions.
  • An understanding of the key drivers of value-based care including clinical outcomes, growth, and Profits & Loss statement Leadership
  • Collaborating for results across a wide array of team members within ACO organization Leadership
  • Effective communication and interpersonal skills, as they develop relationships across the organization Leadership
  • Shift is Monday-Friday standard business hours. No nights, no weekends and no holidays.
Required Qualifications
  • 5+ years of prior work experience managing operational & quality management KPIs in a healthcare setting
  • 2+ year of prior leadership experience
  • 4+ years of analyzing complex data analysis and reporting & deliver presentations to providers
  • Bachelor's degree in business/healthcare or equivalent work experience required.
  • Consulting or business development experience is a plus.
  • Experience and expertise implementing service models that innovate, add value, and drive operational results in health fields.
  • Experience with Episodes of Care or Value Based Care
  • Strong critical thinking skills needed to analyze medical data and reports.
  • Experience with Health Care Organizations
  • Ability to travel 50-60% nationwide
Clinical Licensure Requirement

Eligible candidates must hold one of the followng licenses:
  • RN (Registered Nurse) that must hold an unrestricted license in their state of residence, with multi-state/compact privileges and have the ability to be licensed in all non-compact states
  • LCSW (Licensed Clinical Social Worker)
  • OT (Occupational Therapist)
  • PT (Physical Therapist)
Preferred Qualifications
  • 6+ years of healthcare performance management
  • 3+ years value based care experience
Education
MSW or Registered Nurse (BSN preferred)

Pay Range

The typical pay range for this role is:

$87,035.00 - $187,460.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
