Summary/Position Objectives:
The Housing/Justice Supports and Services Administrator builds collaborative relationships with municipalities, counties, and state housing & justice entities across the state of Michigan. They leverage county housing/justice programs and initiatives to better support the unique needs of our Medicaid members with housing insecurities and needs related to justice involvement. Works directly with Aetna Better Health of Michigan members and community stakeholders and agencies to address both emergent and urgent housing needs and needs related to past and/or current justice involvement. Incumbent will build strategic partnerships and collaborate with Michigan agencies, community stakeholders, and other entities, to address housing and justice needs in the Michigan Medicaid population.
Responsibilities include:
- Develop policies and procedures for supporting member access to the Michigan State permanent supportive housing
- In partnership with the health plan and by utilizing the Michigan Medicaid contract, establish a strategic plan to include objectives, required reporting, intended outcomes and results.
- Serve as health plan liaisons and cross collaborative partners with Michigan state agencies, provider groups, and community-based organizations.
- Provide training and education to both internal staff and external stakeholders in Michigan.
- Support Aetna Better Health of Michigan Care Management & Utilization Management staff, when complex needs are identified, and assistance is needed with connection to services, additional resources, and transition planning.
- Attend health plan rounds which may include transitional rounds
- Partner, support and collaborate with Michigan State and local housing & justice authorities
- As the Aetna Better Health of Michigan Housing/Justice Subject Matter Expert, collaborate with the RFP team and assist with providing information for State RFP requests and renewals
- Emphasize on reductions in care disparities and increased health equity
- In partnership across multiple departments, coordinate the monitoring and tracking of outreach events which align and support community and membership goals
- 5 or more years of experience in public sector, managed health care.
- Minimum of (2) years' experience in assisting the elderly and persons with disabilities to secure accessible, affordable housing through Federal (such as HUD, Shelter Plus Care, SAMHSA, and USDA), as well as local programs.
- Proven track record meeting project dates.
- Minimum of 2 year's experience working with individuals with past and/or current justice needs.
- Broad knowledge of community resources including behavioral health and substance use disorder delivery system in Michigan
- Ability to interpret rules, regulations, and policies and to make decision in accordance with established policies.
- Excellent verbal, written and presentation skills.
- Candidate must be a resident of Michigan
- The highest level of education desired for candidates in this position is a bachelor's degree in Human Services or a related field is preferred, or an equivalent combination of education and experience.
The typical pay range for this role is:
$60,300.00 - $132,600.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
