The health coach will visit a variety of stores associated with our client.The main onsite location is based in Scranton, South Carolina.Working Hours:

Employee will work Monday- Friday 8:00am-4:30pm EST; flexibility is required.There are no weekends or holidays required.The ideal candidate has patient coaching experience, utilizing a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal health Status.

The Wellness Coach Consultant:
  • Assessment of Members:
    • Through the use of clinical tools and information/data review, uses knowledge and experience of lifestyle risk behaviors to conduct comprehensive evaluation of members' health status and lifestyle.
    • Utilizes assessment techniques to determine members' level of health literacy, technology capabilities/access, and readiness to change.
  • Enhancement of Medical Appropriateness and Quality of Care:
    • By assimilating health assessment data analysis, establishes a plan of care based on members' health status/risks, education, and support needs.
    • Partners with members to identify opportunities for integration into available internal/external programs.
    • Using holistic approach consults with supervisors or others to overcome barriers with assisting members in meeting goals and objectives.
    • Assess members using clinical tools and information/data review, conducts comprehensive evaluation of member's needs and benefit plan eligibility for available integrated internal and external programs/services.
    • Using holistic approach consults with supervisors, Medical Directors and/or others to overcome barriers to meeting goals and objectives.
    • Identifies and escalates quality of care issues through established channels.
    • Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
    • Provides coaching, information, and support to empower the member to make ongoing independent healthy lifestyle choices.
    • Interprets and utilizes clinical guidelines/criteria to positively impact members health.
    • Provides up-to-date healthcare information to help facilitate the member's understanding of his/her health status.
    • Helps member actively and knowledgably participate with their provider in healthcare decision-making.
  • Monitoring, Evaluation and Documentation of Care:
    • Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet the member's goals.
    • Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines.
Required Qualifications
  • 3+ years of coaching experience
  • 1+ years of experience in facilitating behavior change with a patient or client.
  • 1+ years of experience in wellness related areas such as
    • Weight management
    • Tobacco Cessation
    • Stress Management
    • Nutrition
    • Physical Activity
    • Health lifestyle behavior change
  • 1+ years of experience working with MS Office applications such as Excel, Word, PowerPoint, and Outlook
  • Ability to commute to/from Scranton, South Carolina OR surrounding areas for onsite work based on plan sponsor's needs. This could be up to 100% of the working week.
  • Ability to work from home Monday-Friday, 8am-4:30pm when travel is not scheduled.
Preferred Qualifications
  • Background in motivational interviewing, active listening skills, transtheoretical model, mindfulness-based and other evidence-based coaching techniques.
  • Health Coach Certification
  • Personal Trainer Certification
  • Call Center experience
  • Demonstrated ability to communicate, problem solve, and work effectively with people, and use good judgement in challenging situations.
  • Ability to work with a multidisciplinary team.
Education
  • Bachelor's Degree in a health-related field (e.g. Health Ed, Wellness, Health Promotion.)

The typical pay range for this role is:$46,988.00 - $91,800.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
