At the 24-Hour Nurse Line the Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status. Requires an RN with unrestricted active license.

- Hours required: Start time will be 10 am EST, 8 hour shift, every other weekend, 4 holidays per year.
- Must reside in a nurse Compact State and hold a Multi State Compact license.
- The 24-Hour Nurse Line provides a telephone-based information and decision support program to eligible members. The Nurse will be responsible for the following:
- Facilitates effective health care decision-making.
- Improves the quality of care.
- Improves member satisfaction with their health care plan.
- Facilitates appropriate use of health care resources.
- Provides up-to-date health care information to help facilitate members' understanding of their health problems and possible options.
- Provides assistance by clarifying available options for the member's particular health problem or issue.
- Helps members actively and knowledgeably participate with their provider in their own health care decision-making.
- Promotes healthy lifestyles to help lower personal health risks, preventing illness.
- Awareness of the member's available health and wellness programs, and makes referrals to these programs as appropriate.
- Active unrestricted RN compact state licensure.
- 3+ years of clinical experience required, with preference for the following areas: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology.
- 1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word).
- Must be open to a start time of 10 am EST, 8 hour shift, every other weekend, 4 holidays per year.
- Willingness to obtain additional state licenses upon hire
- Managed Care experience preferred.
- Previous coaching experience preferred.
- Model candidates have telephone triage nursing experience.
- Associate Degree minimum required.
- Bachelor's Degree preferred.

The typical pay range for this role is:$54,095.00 - $116,760.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
