As a Qualified Behavioral Health Provider (QBHP), you will help with achieving patient wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation.
Primary Responsibilities:
- Coordination of services both within and outside of Life Strategies Counseling ( i.e counseling, medication management, welfare, vocational services)
- Must achieve at least 60% Level of Care on individual and collateral visits
- Must conduct home visits at least once per month with client and guardian
- Provide transportation as needed for clients

Required Qualifications:
- Valid driver's license
- Must be able to travel independently
Preferred Qualifications:
- 1+ years of related experience and/or training
- Solid communication skills
- Ability to coordinate resources
