Standard Hours: Monday - Friday normal daytime business hours (no on-call, no weekends and no holidays required).
This is a field based position in the greater Washington DC area and you will be in the field 100% of the time going into members homes.
Primary Responsibilities:
- Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care in their home
- Manage members through an acute medical situation and work with chronic conditions, including testing, diagnostics, interventions
- Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services
- Manage the care plan throughout the continuum of care
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:
- Associates degree or higher
- Current unrestricted Registered Nurse license in Washington DC
- Certified in Basic Life Support
- 1+ years of experience in post - acute care, such as long-term care
- 1+ years of experience with using an Electronic Medical Record
- Valid Driver's License and Access to reliable transportation
- Ability to travel up to 100% to visit members in their homes within Washington DC
- Located within 50 miles of Washington DC
Preferred Qualifications:
- 1+ years of experience working with the geriatric population
- Field based experience going into patients homes
- Experience creating care plans
- Case Management experience
Washington, D.C. Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
