Hospice RN Job Summary:
The Hospice RN provides compassionate, person-centered care to patients in their homes or designated care settings. Responsibilities include conducting patient assessments, implementing care plans, documentation, coordinating care, and facilitating transitions. The RN works under the direction of the Director or designee and collaborates with the interdisciplinary team (IDT) to deliver holistic hospice care.
Key Responsibilities:
Provide direct patient care, including assessments, symptom management, wound care, and pain management.
Coordinate and manage a caseload of hospice patients, ensuring compliance with hospice policies and regulatory requirements.

Communicate effectively with patients, families, team members, and community resources.
Maintain competency through continuing education and uphold quality care standards.
Participate in meetings, case conferences, and on-call rotations.

Qualifications:
RN License (Montana) and BLS certification required.
Minimum 2 years of acute care, hospice, or related nursing experience.
Strong knowledge of hospice philosophy and regulatory compliance.
Proven ability to manage patient care autonomously and work collaboratively within a team.
Graduate of an accredited nursing program.
