Job Description
The Pappas Rehabilitation Hospital for Children (PRHC), operated by theMassachusetts Department of Public Health (DPH), is seeking a dedicated and compassionate Social Worker III (Case Manager) to join our Case Management team. The Case Manager is responsible for developing, coordinating, and implementing comprehensive treatment plans for children and young adults with developmental and medical needs. This position plays a critical role in supporting families, promoting patient independence, and facilitating smooth transitions back into the community upon discharge.
Schedule
This is a 40-hour per week position 8:00am-4:30pm Monday-Friday, with occasional flexibility based on patient and family needs.

Duties and Responsibilities (This is a general summary and not all-inclusive.)
- Conduct pre-admission interviews and assessments with patients and families; make recommendations to the Admissions Committee.
- Complete and document a psycho-social history for each new patient.
- Provide weekly supervision of interdisciplinary treatment teams to ensure integrated clinical service plans.
- Coordinate, monitor, and assess treatment programs to ensure they meet individual goals and compliance standards.
- Evaluate psychosocial, legal, family, behavioral, and economic issues impacting care planning and treatment outcomes.
- Lead the care planning process, regularly reviewing data to adjust interventions and treatment goals.
- Assist families with discharge planning, exploring community resources such as housing, education, and medical services.
- Facilitate patient/family education on treatment, services, and transition planning.
- Coordinate and provide transportation and chaperoning to appointments (e.g., housing, assessments, foster care reviews).
- Facilitate unit-based educational meetings to help patients build financial, organizational, and life skills.
- Supervise and evaluate social work interns during their clinical placements.
- Identify and implement opportunities for service delivery improvement, participating in quality improvement initiatives.
- Maintain compliance with all hospital, state, and federal regulations.
- Attend required annual orientation and ongoing training programs.
- Serve as a liaison among families, school staff, medical providers, and community service agencies.
Required Qualifications
- Knowledge of social work principles and casework practices.
- Experience with interviewing techniques and psychosocial assessments.
- Strong skills in record keeping, report writing, and documentation.
- Ability to interact effectively and empathetically with patients and families from diverse backgrounds.
- Capacity to maintain professional judgment and calm demeanor in stressful or emergency situations.
- Ability to work collaboratively in interdisciplinary teams and motivate clients.
- Understanding of developmental disorders, mental health symptoms, and psychosocial needs.
- Excellent communication and organizational skills.
Preferred Qualifications
- Experience working in a pediatric, rehabilitative, or developmental care setting.
- Familiarity with community-based support services, including housing, education, and health care systems.
- Experience supervising clinical staff or social work interns.
- Bilingual abilities are a plus.

Qualifications

MINIMUM ENTRANCE REQUIREMENTS:
Applicants must have (A) a Master's degree in social work, psychology, sociology, counseling, counseling education or criminal justice or a relevant human services degree and (B) a current and valid Licensures as a Licensed Social Work Associate, Licensed Social Worker, Licensed Certified Social Worker or Licensed Independent Clinical Social Worker issued by the Massachusetts Board of Registration, and (C) and three (3) years of full-time or equivalent part-time experience in social work, or (D) or any equivalent combination of the required experience and the substitutions below.
Applicants at the Department of Children and Families must obtain the required license in Social Work within the first nine (9) months of employment.
The classification may require possession of a current and valid Motor Vehicle Driver's License at a class level specific to assignment.
Substitutions:
I. A Doctorate degree in a related field may be substituted for two (2) years of the required (C) experience.
Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
