Job Description

The Program Coordinator II, Community Systems Project Coordinator is responsible for the development, implementation, and coordination of community resources. Priority areas of focus include, but are not limited to housing, assistive technology, program development, and transportation. Development and data tracking with include regular work with Regional and Area program and contracting staff.

Duties and Responsibilities (these duties are a general summary and not all inclusive)

  • Serve as a resource specialist to four (4) Area offices regarding a broad range of housing opportunities and funding sources for agencies as well as individuals served.
  • Travel occasionally between the four (4) Area offices in the Metro Region and provider sites to collaborate in planning and reviewing services, and to provide training, consultation, and resources.
  • Develop and maintain resources that identify organizations and programs that assist with housing needs, eligibility requirements, set-aside units, and grant opportunities available through state and federal government, private organizations/trusts, and commercial opportunities.
  • Work with four (4) Area offices to maintain updated lists and current information for individuals interested in housing so timely applications can be submitted as opportunities become available.
  • Explore housing options and roommate options for individuals supported throughout the Metro Region, including individuals with ASD.
  • Oversee and monitor regional consultant housing contract.
  • Serve as the Regional lead for the Assistive and Supportive Technology, and Remote Supports and Monitoring initiatives by increasing Area and Regional staff's understanding of needs, opportunities, providers, and equipment.
  • Participate in statewide Assistive Technology meetings, develop an understanding of provider expertise and capacity, monitor and track referrals, related budgets, and spending.
  • Co-lead annual Technology Fair.
  • Lead program development planning activities and maintain program development tracking for the Metro Region.
  • Work with Area offices, transportation entities, and service providers to coordinate and arrange for cost-effective, efficient transportation for individuals in DDS-funded day supports.
  • Expand the use of alternative transportation modalities for individuals going to community jobs, day services, recreational activities, etc.
  • Develop and maintain data tracking systems for housing, assistive technology, program development and transportation initiatives.
  • Develop, generate, and distribute Regional and Area management reports for key initiatives.
  • Perform related duties as assigned.

Required Qualifications

Qualifications

First consideration will be given to those applicants that apply within the first 14 days.

MINIMUM ENTRANCE REQUIREMENTS: Applicants must have at least (A) three years of full time, or equivalent part-time, professional, administrative or managerial experience in business administration, business management or public administration the major duties of which involved program management, program administration, program coordination, program planning and/or program analysis, or (B) any equivalent combination of the required experience and the substitutions below.

Substitutions:

I. A Bachelor's degree with a major in business administration, business management or public administration may be substituted for a maximum of two years of the-required experience.

II. A Graduate degree with a major in business administration, business management or public administration may be substituted for the required experience.


III. A Bachelor's or higher degree with a major other than in business administration, business management or public administration may be substituted for a maximum of one year of the required experience.

Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.

Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
