Job Description

The Department of Mental Health (DMH) is seeking a qualified professional for the position of Human Services Coordinator II in Fitchburg to provide oversight of case managed youth that are in statewide level of care, DMH community services, as well as a liaison to community providers regarding services to DMH clients.

The selected candidate assists the Child, Youth and Family (CYF) leadership team in developing plans for the service system, coordinates referrals from Service Authorization to providers and case managers, provides supervision to case management, oversees all authorized youth within the site, and serves as a DMH System Integration Specialist (SIS) and liaison to Community Service Agencies (CSAs).

The schedule for this position is 8:30 AM to 5:00 PM Monday-Friday with Saturday and Sunday off.

Duties and Responsibilities (These duties and responsibilities are not all inclusive)

  • Provides supervision to case managers' caseloads and attend treatment team meetings as needed.
  • Provides direct assistance to clients by helping them to obtain needed services to enable them to transition to and/or remain in the community.
  • Facilitates transition of new cases from service authorization by completing referrals, and other necessary documentation.
  • Prioritizes and monitors utilization and communicates to Director and/or Assistant Director of CYF Services.
  • Provides oversight and service management for non-case managed clients.
  • Supervises case management staff ensuring DMH regulations are being followed, scheduling is approved/set, mileage reimbursement and completion of performance appraisals (EPRS).
  • Provides overall risk management for all authorized youth.
  • Consults with service providers to facilitate provision of appropriate, quality care treatment and rehabilitation.
  • Facilitates a collaborative working network of providers
  • Crisis intervention related tasks
  • Provides relevant information to CYF Leadership on client and programmatic issues that impact upon service delivery; identifies service gaps, strengths and limitations in local service area.
  • Participates with the CYF Leadership team in planning and development of case management practices and procedures and RFR development, as needed.
  • Collaborates with the CSA director or designee.
  • Provides education to CSA staff about DMH services, including clinical criteria and specific service authorization criteria.

Required Qualifications:

  • Knowledge of the principles and practices of clinical and social psychology and psychosocial rehabilitation.
  • Familiarity with the organization and functions of community mental health services.
  • Ability to gather information through questioning and observing individuals and by examining records and documents.
  • Willingness to travel for job-related purposes.

Preferred Qualifications:

  • Behavioral health experience
  • Strong and effective interpersonal skills, including the ability to listen well, relate to others and demonstrate sensitivity and respect for individuals.
  • Demonstrated commitment to serving a diverse population with cultural competence and sensitivity, as well as the ability to work with a wide range of constituencies with diplomacy and tact.
  • Proficiency in Microsoft Office applications including Outlook, Word, and Excel.
  • Given the population served, bilingual or multilingual fluency

Working Conditions:

This position travels to meet with clients and family members outside the office in various settings such as homes, hospitals, programs, and other accessible community settings. Candidates will need a valid driver's license and access to a reliable motor vehicle for transportation.

Qualifications

This requisition will remain open until filled; however, first consideration will be given to those applicants that apply within the first 14 days.

Applicants must have (A) at least four (4) years of full-time, or equivalent part-time, case management or clinical/therapeutic experience (B) any equivalent combination of the required experience and the substitutions below.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Substitutions:

I. A Bachelor's degree in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.
