As the Care Coordinator for Children and Youth With Special Health Needs, you will orchestrate the formulation and execution of comprehensive, developmentally suitable individual service plans, ensuring a proactive engagement with stakeholders that include parents, healthcare professionals, educational institutions, and service providers. Your leadership role extends beyond individual care to driving systemic enhancements within the realm of CYSHN care, aligning with the principles of the Medical Home Model. Engaging in outreach initiatives and training programs will be second nature to you, targeting parents, community-based service providers, and educators to promote an understanding of legislations, policies, regulations, entitlements, benefits, and available services and resources. This role actively fosters family involvement by nurturing culturally and linguistically proficient leadership and training opportunities, emphasizing the importance of encouraging and enabling families to engage in the process. For transition-age clients, aged 14-22, your integral role involves developing comprehensive healthcare and life transition plans while closely collaborating with various stakeholders to ensure inclusivity across diverse cultural, linguistic, and economic backgrounds within all facets of service provision.

In addition to these impactful responsibilities, diligent record-keeping is crucial for monthly reporting tasks and the evaluation and enhancement of care coordination programming. You will actively participate in outreach, training initiatives, meetings, and other administrative or program activities as directed by the Director of the Care Coordination program and the Director of the Division for CYSHN. Duties and Responsibilities (these duties are a general summary and not all inclusive):

  • Collaborate with state agencies, service providers, parents' organizations, private vendors, and clinical professionals to efficiently coordinate and enhance service delivery for children and youth with special health needs (CYSHN).
  • Participate actively in multidisciplinary and multi-agency teams, contributing expertise to ensure comprehensive support for CYSHN.
  • Provide direct support, comprehensive technical assistance, consultation, and training to both families and professionals on public resources and entitlements, ensuring a clear understanding of eligibility criteria and regulations for CYSHN.
  • Work in partnership with families and service providers to identify and advocate for the needs of CYSHN, maximizing integration of parent input into the Division's programs.
  • Refer families to appropriate Division programs, state agencies, and community-based organizations, following established guidelines and facilitating referrals to educational, residential, clinical, and social services providers, as well as financial resources.
  • Document all interactions with families and other providers meticulously and in a timely manner using the designated data system, adhering to program protocols.
  • Actively engage in supervisory, team, and other required meetings to foster collaboration and ensure alignment with program goals.
  • Orchestrate the formulation and execution of comprehensive, developmentally suitable individual service plans for CYSHN, maintaining a proactive engagement with stakeholders, including parents, healthcare professionals, educational institutions, and service providers.
  • Drive systemic enhancements within the CYSHN care domain, aligning initiatives with the principles of the Medical Home Model.
  • Develop and implement outreach initiatives and training programs targeting parents, community-based service providers, and educators to promote understanding of legislations, policies, regulations, entitlements, benefits, and available services and resources for CYSHN and their families.

Required Qualifications:

  • Knowledge of development disorders, developmental disabilities, and/or mental health.
  • Knowledge of basic clinical skills and understanding of human behavior, social work, healthcare, and law.
  • Working knowledge of available community resources and programs.
  • Knowledge of basic clinical skills and understanding of applicable human behavior, social work, healthcare, medical terminology, and law.
  • Excellent communication, decision making, and conflict resolution skills.
  • Ability to plan, assign, and coordinate the activities of others while engaging in team building and growth.

Preferred Qualifications:

  • Advanced Public Health knowledge, studies, practice, and/or training. Minimum of three (3) years' experience in the field.
  • Bilingual, bicultural in Spanish.
  • Lived experience preferred.
  • Experience conducting intake assessments.
  • Proven success in interagency collaboration and working with families of children with medical complexity.
  • Ability to work both independently and with others under time pressures and deadlines.
  • Strong relationships with and knowledge of community-based organizations and other state and community agencies.
  • Proficient with Microsoft Office applications including Excel, Word, Teams, and Outlook.
  • Experience working with Windows-based applications and data management systems such as Electronic Medical Records (EMR).
  • Must have a valid driver's license and regular access to a vehicle.

Qualifications

Applicants must have (A) at least four (4) years of full-time, or equivalent part-time, case management or clinical/therapeutic experience (B) any equivalent combination of the required experience and the substitutions below.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.

Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.
