Job Description

The Department of Mental Health (DMH) is seeking a talented and experienced human services professional, who will serve as an Adult Case Manager for clients originating from ten cities/towns in Southeastern Massachusetts. The successful candidate will provide assessment, outreach, support, service coordination and person-centered advocacy to DMH eligible adults by promoting the agency's philosophy of recovery, person-centered engagement and cultural competence. The selected candidate will utilize strength-based goal setting and foster independent decision-making for persons served. Additional responsibilities include the development, implementation and monitoring of individual service plans and assisting individuals in securing services while performing all duties to reflect the highest level of respect and dignity for the persons served.

Please Note: This position is 40 hours per week. The selected candidate will work Monday through Friday from 8:30am - 5:00pm. The days off are Sunday and Saturday.

Duties and Responsibilities (these duties and responsibilities are not all inclusive):

  • Review eligibility determination record and obtain clinically relevant information through medical and psychiatric records, mental health assessments, school records as well as interviews with persons served & service providers
  • Develop Comprehensive Assessment and treatment plan (ISP), reviewing treatment progress regularly and making modifications to the ISP as necessary
  • Schedule multidisciplinary team meetings to formulate recommendations for treatment and services & maintain consistent and effective collaboration and communication with service providers and colleagues
  • Conduct in-person visits & maintain regular communication with clients, service providers, legal guardians and family members
  • Make referrals for services when necessary
  • Ensure that updated collateral information is included in client chart & notes are written on all significant contacts, including but not limited to, face to face visits, phone contacts with the client's family/guardian, incidents involving a client reported from a third party & all crisis events
  • Maintain both electronic and paper medical records with up-to-date information, reflecting current bio-psycho-social information on persons served
  • Participate in meetings and case conferences

Required Qualifications:

  • Demonstrated skills in the principles, practices and techniques of counseling, interviewing, engagement, motivation and reinforcement
  • Experience with the diagnoses and symptoms of mental and emotional disorders & the programs and services available to this population
  • Expertise in the methods of general report writing
  • Knowledge of principles and practices of crisis intervention, risk mitigation and group process for counseling the mentally ill
  • Ability to prioritize and multitask duties

Special Requirements:

Travel is required - This position directly manages cases in the DMH Quincy Site catchment areas; however, frequent travel is required throughout the DMH Southeast Area and occasional required statewide travel to various acute inpatient hospitals or other treatment settings. Current and valid Massachusetts Class D (or equivalent from another state) Motor Vehicle Operators license required.

Preferred Qualifications:

  • Knowledge of the psychosocial problems of those with mental/emotional, substance use and co-occurring disorders
  • Given the population served, bilingual or multilingual fluency. Fluency in Mandarin and/or Vietnamese, preferred.

Qualifications

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
