Job Requirements: 

  • FL RN (Registered Nurse) license
  • Education: Associates Degree in Nursing
  • Experience: Minimum of three years of Acute Care RN experience within a hospital setting preferred.
  • Minimum 2 Years of RN Case Management within a Large Acute Care “Hospital” preferred.
  • 1 year of Utilization Review experience preferred

About the Job:

  • Working closely with the collaborative care team, the case manager assesses plans, intervenes, and reassesses an individual’s needs to implement a plan of care to meet the needs of that individual
  • The case manager assesses patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care.
  • Utilization review is conducted according to ShorePoint Health guidelines
  • Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge
  • Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. 
  • Day Shift Available (8X5's varying days)

  • Established leadership skills. Proven critical thinking skills. Ability to collect, organize, and analyze data. Innovative and creative approach to work. Computer literacy. Ability to work independently and in stressful situations. Ability to demonstrate effective written and oral communication skills.
  • Minimum five years of clinical e experience in an acute care setting.
Preferred
  • Previous case management and/or utilization management/utilization review experience.

Education

Required
  • Associates or better in Nursing
  • High School or better
Preferred
  • Bachelors or better in Nursing or related field

Licenses & Certifications

Required
  • L-FL-RN License
Preferred
  • C-AHA BLS Basic Cardiac
