Job Description
Performs various aspects of hospital care within assigned patient care unit under direct supervision of a Registered Nurse.
Job Responsibility
- Provides direct patient care as defined in the New York Nurse Practice Act in the Home, Nursing Facility or Inpatient or Residential Setting.
- Applies hospice and nursing practice standards to patient care activities.
- Implements plan of care initiated by the Hospice RN.
- Provides accurate and timely documentation of patient services reflective of plan of care.
- Reports changes in patient condition or needs to clinical staff.
- Assess and provides patient and family/caregiver education and information.

Job Qualification
- Graduate of an accredited school of practical nursing or of a Nursing Vocational or Nursing Technical school required.
- Current license to practice as a Practical Nurse in New York State required, plus specialized certifications as needed.
