About the team:
We are looking for a mission-driven health coach to join our team to provide world-class care delivery services to Maven members. Our Care Delivery team consists of care coaches and care advocates who help our members navigate digital care and access the tools to unlock their best experience and outcomes. Our team works to continuously learn more about members and ourselves, and you’ll move fast to adapt and grow as we implement data-driven strategies for improving the lives of our members through a shared coaching model. As a Care Coach, you’ll coordinate care for our members beyond the digital space. Through goal setting and program opt-in, you’ll drive behavior change and improved outcomes for higher risk members.
Our Care Delivery team works remotely and supports Maven members 24/7/365. We are currently hiring for the following shifts: Monday - Friday 7AM - 330PM EST or Monday - Friday 11AM - 730PM EST
As a Care Coach, you will be responsible for:
- Conveying the benefits of the Maven platform and engaging in care coaching to our Maven members, optimizing opt-in rates for care programs
- Utilizing coaching framework and principles to drive the creation of SMART goals with members, assuming responsibility for adherence and refinement of goals within Maven’s clinical program design
- Conducting introductory and follow-up appointments in succession, establishing a longitudinal relationship through outreach and caseload management
- Adhering to clinical program guidelines and protocols, utilizing motivational interviewing skills and knowledge of behavior modification techniques to influence behavior change and promote optimal outcomes
- Moving fast to assess member knowledge and barriers, using a personalized approach in providing health education to members of varying ages, backgrounds, cultures and literacy levels
- Collaborating with providers of the member’s care team, incorporating recommendations to update and manage member care plans
- Remotely monitoring member health data and activities, leading with data and following protocols and procedures to take action
- Ensuring seamless access to care through coordination of care within the Maven digital network as well as in-person networks and providing hand-offs to external parties as needed
- Embracing a continuously learning mindset by participating in and leading efforts to improve quality of care and clinical outcomes
- Acting as a team player and providing coverage to ensure all members across the Maven platform receive appropriate care and responses when needed
- Collaborate closely with cross-functional stakeholders across our Care Delivery, Customer Support, Provider group, Product, and Clinical Innovation teams
- Anticipate and address member’s social determinants of health needs
- Any other duties as assigned
Minimum Qualifications:
- 1- 2+ years experience in health coaching
- Ability to remain flexible with changes and have a growth mindset
- Ability to communicate with patients empathetically in writing and verbally, and passionate about creating a positive member experience with optimal outcomes
- Experience achieving KPIs relevant to patient outcomes, including opt-in rates, follow-up adherence and member satisfaction
- Health coaching certification (NBC-HWC or ICF), or personal training certification
- Proficiency in navigating new technologies and systems as part of working on a remote team
Preferred Qualifications:
- Experience in family planning and maternal-child health areas such as pregnancy and fertility, specifically serving high-risk members
- Clinical practice background in nursing or behavioral health
- Experience in a start-up or fast-paced environment with ability to navigate systems quickly and adapt to change
- Comfort with technology tools like Google suite, CRM platforms, EHRs, and Zendesk
For candidates in NYC, CO, or CA, the base salary range for this role is $56,000-$68,000 per year. You will also be entitled to receive stock options and benefits. Individual pay decisions are based on a number of factors, including qualifications for the role, experience level, and skillset.
