Description and Requirements
Primary Responsibilities
- Assess community health needs and recommend / prioritize educational programs
- Support in the development and execution of health educational programs
•Support the creation of objectives and goals
•Work with internal team to develop relevant content / materials and facilitate work shops, training sessions and community events
- Work with team lead and project associate to track identified metrics and monitor progress
- Program effectiveness
- Work with clinical and care management team to develop culturally sensitive and competent educational materials and resources
- Work to promote healthy behaviors in the community (based on area of focus)

Essential Skills
- High school diploma or GED Equivalent
- Certificate in Health education (Certified Health Education Specialists) or BA Public Health, Health Education or Related Field
- Bi-lingual - Spanish
- Should be a community health worker
- Experience working with diverse communities, including faith-based organizations and Local healthcare providers
- Strong communication, interpersonal and presentation skills
- Knowledge of health education principles, practices and cultural competency
- Program planning and evaluation
