- Meaningful Impact: Make a significant difference in the lives of patients and their families by providing compassionate end-of-life care.

Key Responsibilities:

Team Leadership: Directly supervise a regional team of Care Coordinators, providing oversight of day to day operations and guidance navigating insurance benefits, community resources and support services (30% of the time)

Lead Social Worker: Work directly with patients and families (70% of the time)
  • Patient and Family Support: Provide emotional and psychosocial support to patients and their families, helping them navigate the complexities of end-of-life care.
  • Care Planning: Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
  • Resource Coordination: Connect patients and families with community resources and support services, including financial assistance, counseling, and respite care.
  • Advance Care Planning: Assist patients and families with advance care planning, including living wills, healthcare proxies, and other legal and medical decisions.
  • Education and Advocacy: Educate patients, families, and caregivers about the hospice philosophy and available services, advocating for patient needs and preferences.
Qualifications
  • Education: Master's degree in Social Work from an accredited institution.
  • Licensure: State licensure as a Licensed Clinical Social Worker (LCSW) preferred
  • Experience: At least 2 years of clinical experience, preferably in hospice, palliative care, or end-of-life settings.
  • Skills: Strong interpersonal and communication skills, with the ability to provide empathetic and non-judgmental support.
  • Knowledge: Familiarity with end-of-life care principles, grief counseling techniques, and community resources.
  • Attributes: Compassionate, self-directed, and able to handle emotional and challenging situations with grace and professionalism.
  • Travel: Must be able to travel regionally as needed to visit patients.

Salary/ Wage Range
$56,500.00 - $83,000.00 / year

Compensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience and may fall outside of the range shown.
