General Purposes of Job:
DaVita is a rapidly growing Fortune 500 Healthcare Services company and a leading provider of kidney care in the United States. The goal of this role is ensure DaVita is the provider of choice to hospitals. This teammate will engage renal discharge planners and case managers in an effort to build and strengthen relationships. Their focus is to ensure a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients to ensure a successful transition to the outpatient setting.

Essential Duties
- Consistent outreach to discharge planners to provide education and build awareness of offerings
- Identify opportunities to grow as provider of choice through relationship building and addressing feedback
- Partner with internal field teammates to optimize the admission process and implement best demonstrated practices
- Assist with necessary communication between hospital, admission and clinic teammates to work through patient placements with extenuating circumstances
- Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers
- Develop comprehensive knowledge of local market

- Ability to build and foster strong relationships with people across many levels and diverse backgrounds

- Bachelor's Degree preferred
- Ability to travel to various locations to meet with partners (internal and external) on an as needed basis. Includes valid driver's license in state of residence and insurable driving record
- Intermediate computer skills with a variety of software packages including but not limited to Microsoft Word, Excel, PowerPoint, billing systems

Salary/ Wage Range
$68,000.00 - $115,000.00 / year
Compensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience and may fall outside of the range shown.
