Nutritional Assessments: RDs evaluate patients' nutritional status, including reviewing medical history, lab results, and dietary habits. They assess things like protein intake, fluid balance, and overall nutrition to tailor dietary recommendations. Developing Individualized Diet Plans: Based on the assessment, dietitians create customized nutrition plans to help patients manage their kidney disease. This can include controlling protein, phosphorus, potassium, and sodium levels, depending on the patient's needs. Education and Counseling: They educate patients about how diet affects kidney function and teach them how to make food choices that will support their treatment plan. RDs provide guidance on meal planning, portion sizes, and how to read food labels. Monitoring and Adjusting Plans: Dietitians track a patient's progress over time, adjusting nutrition plans based on lab results, treatment changes, or shifts in the patient's health status. Support for Dialysis Patients: For those on dialysis, dietitians help manage the specific dietary needs that arise, like ensuring adequate protein intake or controlling fluid retention
