In addition, this role will partner with regional leaders, particularly the Population Health Director and Senior Clinical Informatics Specialist, to indirectly support daily huddle and CDR workflow optimization. CDSs are trained to be experts in structured clinical assessments, accurate and specific documentation, population health workflows, and team based care.Core Responsibilities:

- Chart Reviews
- Access and review HIE/EMRs to identify potential suspects based on test results, etc.
- Identify the need for Medical Records requests from Blue Button and other payer data
- Prepare and lead monthly Rejected Suspect Reviews
- Partner with Population Health Director on general review support
- Identify and analyze opportunities for improving patient quality outcomes and cost of care
- Analyze chronic condition prevalence based on patient demographics and identify opportunities for interventions
- Monitor clinical documentation efforts
- Other duties, as assigned
- Ease and expertise working with multiple technical systems, including Greenway, Canopy, and Excel
- Ability to collaborate across multiple teams and engage in multiple perspectives to work toward a common goal
- Proven experience and ability to build relationships with Providers
- Proficiency in tracking down and leveraging external resources, such as hospital records, Blue Button, and payer data, to support retrospective and prospective accurate documentation
- Ability to succeed in a fast-paced, ever-changing environment
- Self-starter with a high degree of drive, initiative, and follow through
- Ability to develop, plan, and implement short and long term goals

The typical pay range for this role is:$18.50 - $38.82This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
