- Own the execution and the ongoing refinement of select Population Health programs
- Work closely with regional leaders to assist with program implementation and execution
- Work side by side with other central leaders and teams to ensure effective design and implementation planning including Data, Product, Managed Care Operations, Clinical Operations & Learning, etc.
- Accountable to key population health metrics, including quality measures, utilization measures, and financial measures
- Other duties as assigned.
- 4+ years of applicable experience in healthcare / population health / working with healthcare providers; or other similar fields
- Bachelor's degree required
- Graduate degree preferred but not required (MBA, MPH)
- Ability to build the trust and confidence of a wide range of stakeholders
- Strong leadership and influencing skills
- Incredible level of organization and detail orientation
- Analytical and data savvy. Ability to develop business cases, ROI analysis, and financial analysis
- Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives
- Expert in Excel, Google Apps, project management

The typical pay range for this role is:$82,940.00 - $182,549.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
