Title: Senior Associate, Care Model Strategic Initiatives (Population Health)
Location: Downtown Chicago (30 W. Monroe St.)Role Description:The Senior Associate, Care Model Strategic Initiatives supports the daily implementation of enablement initiatives aimed at innovating Oak Street Health's care model. In this role the Senior Associate will collaborate with other elements of the Population Health team, Field Leadership, and external consultants to stand up enablement functions that support the organization in its efforts to provide better care to our patients. The Senior Associate is responsible for managing the development and updates of technology tools to support these initiatives. The role will partner with Clinical leads to develop/refine workflows to ensure quality and efficiency in operations and ensure operations support program success.Core Responsibilities:

- Support Oak Street Health in navigating industry MLR headwinds on strategic projects that impact our care model
- Support in transitioning to the new CMS hierarchy model, and build programs to implement innovative prospective suspecting capabilities
- Liaise and communicate to Product Team on new care model initiatives
- Support multiple project plans to ensure efforts stay on track for deadlines and prepare updates and read-out on project progress
- Manage technology, dashboards and metrics related to new initiatives and partner with Field leadership to seek opportunities for improvement
- Collaborate with all corporate departments and clinic teams to identify opportunities for improvement and implement solutions
- Other duties, as assigned
- Proven project management skills

- Outstanding written and verbal communication skills and customer service orientation

- Bachelor's degree
- Experience in healthcare operations preferred, but not required

The typical pay range for this role is:$46,988.00 - $112,200.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
