For more information, visit http://www.oakstreethealth.com .Role DescriptionAt Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care.The Population Health Manager will be instrumental in coordinating and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve quality. The role will be an effective cross team leader and support, working side by side with providers, care teams, operations team members, and Treehouse team members.Content areas for the Manager will include, but not be limited to, pharmacy and medication adherence, HEDIS / Stars, and accurate risk adjustment strategy. This will require building relationships with our clinicians and field team members.This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently within an Oak Street division. This role will be aligned with a geographic division to rapidly drive results.This role will report directly to the Regional Vice President or Population Health Director/Regional Director of Operations; with a dotted line to the central Population Health team.Core Responsibilities:

- Be the technical expert on all things clinical documentation and Quality.
- Work side by side with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.
- Support the execution and the ongoing refinement of Population Health programs in the local division.
- Work closely with functional leaders in the central office to assist with strategy and program design.
- Accountability to key population health metrics including quality and financial measures.
- Other duties, as assigned.
We're looking for a motivated leader with:
- Minimum of 3 years of applicable experience in healthcare / population health / working with healthcare providers; or other similar fields
- Ability to build the trust and confidence of a wide range of stakeholders
- Leadership skills to delegate tasks and provide guidance to help team members execute on population health priorities
- Problem-solving and critical thinking abilities
- Incredible level of organization and detail orientation
- Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives
- Proven skills and experience in Excel, Google Apps, project management
- Pleasantly persistent approach
- Humility
- Past scribing experience is a plus, but not required
- Quick learner comfortable with a high level of ambiguity
- Bachelor's degree required

The typical pay range for this role is:$46,988.00 - $112,200.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
