The Senior Population Health Manager helps partners improve healthcare quality and achieve ACO Shared Savings. The Population Health Manager achieves these aims by carefully tracking performance metrics, serving as a project manager, and training partners to effectively implement CVS Accountable Care Foundational Programs such as team-based care, annual wellness visits and proactive primary care. The Senior Population Health Manager facilitates calls with primary care practices, builds strong partner relationships and identifies opportunities where additional subject matter expertise should be deployed.

Essential Duties and Responsibilities: To perform this job successfully, the Senior Population Health Manager must be able to perform the following satisfactorily; other duties may be assigned. • Tightly manage key performance indicators for strategic/large primary care practices.
• Deliver and coach partners on Foundational Programs including patient messaging, same day visit processing, standing orders, pre visit planning, team based documentation, coding and documentation fundamentals, quality outreach campaigns, annual wellness visit workflows and use of patient summaries.
• Host and conduct Action Planning calls with partners to support the implementation of population health priorities as determined in the JOC meetings.
• Provide project management and oversight to guide partner's ACO work.
• Serves as the first escalation point for partner questions and concerns.
• Effectively communicates business and technology issues as well as solutions.
• Build and maintain strong organizational relationships at CVS Accountable Care as well as within partner organizations.
• Integrate population health software (CVS Analytics Platform) into partner interactions and assist partners to optimize their utilization of the platform to meet ACO goals.
• Gather data and track relevant performance metrics. Use this data to drive partner discussions around improvement opportunities and progress towards goals and objectives. Escalate concerns to manager and regional team in a timely manner.
• Document partner progress and barriers and connect partners with SMEs as needed.
• Access and promote resources provided by subject matter experts on quality reporting, promoting interoperability, ACO compliance, value-based payment policy and other related programs.
• Attend weekly regional huddles as well as partner facing events like Symposium.
• Identify and promote CVS Accountable Care training content related to a specific partner's needs.
• Holds partners accountable to goals set in the JOC related to healthcare quality and ACO shared savings.
• Stay current on evolving ACO program requirements from CMS as well as CVS Accountable Care.
• Serve as a mentor to internal team members (Population Health Manager) via one on one interactions and broad team trainings.
• Develop content and speak at partner facing events (in-person and virtually).
• Travel up to 10-25% to partner and internal meetings

Education/Experience:
• Bachelor's degree from an accredited university or college is required.
• Minimum five years' experience in provider clinic, health system, account management, partner services or consulting.
• Master's degree is a plus (e.g., MPH, MHA, MPA, MBA)
• Previous experience in project management preferred.
• Previous experience working with Accountable Care Organizations preferred.

Pay Range
The typical pay range for this role is:
$46,988.00 - $102,000.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
