Position Summary

he Health Coach telephonically outreach es our Medicare population to improve m edication adherence and/or HEDIS diabetes and blood pressure control , provid ing education and address ing any barriers to adherence by utilizing the member's benefits. This may include outreach to pharmacies and pr escribers. TheHealthCoach utilizes acollaborativeprocessofassessment, planning, implementation, and coordination toengage,educate,and promote/influencemembersindecisionsrelatedtoachievingand maintaining optimal healthstatus.

- AssessmentofMembers:Through motivational interviewing and theuseof various tools,uses healthcare knowledgeandexperienceof health risk behaviorstoconductevaluationofmember'shealthstatus for either HEDIS health screening / control or Medication Adherence.

- Utilizesinfluencing/motivationa l interviewingskillstoensure memberengagementand discern health needs based on key questions and conversation to promote behaviorchangestoachieve optimum levelofhealth

- Partnerswithmembersto identify opportunitiesforintegrationinto availableinternal/externalprograms

-Usingholisticapproachconsultswith Manager orotherstoovercomebarrierswithassistingmembersin meetinggoalsand objectives

- Utilizing interviewingandactivelisteningskills,providescoaching, information and supporttoempowerthemembertomakeongoingindependenthealthy healthcare choices.

-Interpretsand utilizes guidelines /criteriato positively impact membershealth

-Providesup-to-datehealthcare informationtohelp facilitate themember'sunderstandingofhis/herhealth status

-Helpsmemberactivelyandknowledgably participate withtheir providerinhealthcaredecision s

- Applies judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex health and social indicators which impact healthcare access, services and resolution of member issues.

-Develops a proactive course of action to address issues presented to enhance the short and long term health outcomes as well as opportunities to enhance a member's overall wellness through integration.
Required Qualifications
1-3 years Microsoft Office
3-5 years practice experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology
-Managed Care and Medicare experience

Preferred Qualifications

Healthcare licensure and/or certifications (RN, LPN, MSW, LCSW , CDE etc ) h ighly preferred

- 5 years practice experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric , Oncology

- Managed Care and Medicare experience

- Previous telephonic coaching , educati on or case manag e ment experience
- ExcellentcomputerskillsincludingMSOfficeapplications
- Independent worker
- Excellent time manager
- Excellent analytical and problem solving skills
- Effective communications, organizational, and interpersonal skills , Bilingual a plus

- Demonstratedabilitytocommunicate,problemsolve,andworkeffectively withpeople,usegoodjudgmentinchallengingsituations

- Experiencein facilitating clientpositivebehavioralchange

- Backgroundinmotivationalinterviewing,activelisteningskills.

Education
Associates Degree or equivalent experience

Pay Range

The typical pay range for this role is:

$46,988.00 - $122,400.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
