Candidate must reside in Kentucky Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through telephonic and in-person outreach and education. CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other.Job Duties include but are not limited to:

- Identify gaps and opportunities to strengthen systems of care within the member's community
- Assist members in utilizing community services, including scheduling appointments with social services agencies (including transportation vendors), and assisting with completion of applications for eligible programs.
- Assist with care plan implementation and member education during telephonic and in-person outreach, help develop care management strategies, and work with team members to provide linkages to programs/resources for the various health and social needs of members.
- Teach disease self-management (i.e. nutrition, symptom tracking and reporting).
- Assist with patient medication adherence by: instructing the member on current medication list, reviewing medications with member and assist in obtaining refills.
- Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner (e.g. progress notes, letters), and other administrative responsibilities as needed.
- Travel to client homes, community locations, various agencies, and other outreach destinations.
- Requires the ability to travel up to 70% of the time to multiple office locations and home locations, occasional weekend hours may be required.
- Travel is with personal vehicle. Must possess reliable transportation and proof of vehicle insurance.
- Must possess a Kentucky CHW certification.
- Demonstrated proficiency with personal computers, keyboard, multiple systems navigation, and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
- 3-5 years relevant experience in serving the Medicaid Populations in urban or rural environments with familiarity of local formal and informal resource networks preferred.
- Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPPA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions.
- Completed and approved CHW/Promotora certification program.
- High School Diploma or G.E.D.
The typical pay range for this role is:$18.50 - $31.72This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
