This position is responsible for member-facing, telephonic clinical outreach and market engagement to support Medicare Part D Stars initiatives. This position will coordinate, plan, and implement quality initiatives for the clinical pharmacy team supporting Medicare Stars.Responsibilities of this position include:

  • Oversee and execute clinical pharmacy programs
  • Coordinate strategies for increasing adherence
  • Utilize motivational interviewing and health coaching to impact member behavior
  • Ensure effective collaboration with internal and external matrix partners to support clinical programs
  • Work collaboratively with leadership and management to strengthen business and operational processes
  • Provide clinical pharmacy support to programs targeted toward the Medicare membership
  • Other duties as required
This position may involve travel up to 10% as needed.Required Qualifications
  • Current unrestricted pharmacist license in the state in which you reside
  • Residing in a state that allows remote Pharmacist practice
  • Demonstrated experience working at a health plan managing Medicare Stars to improve quality outcomes for members. This must include established experience in functions beyond prior authorization approval or drug utilization review. OR 3+ years in a Clinical Pharmacist Role which must include at least one of the following:
    • Rounding and making therapy recommendations as an active part of a multi-disciplinary care team
    • Independently managing patient therapy under a collaborative practice agreement
    • One year of a Pharmacy Practice Residency
Clinical Pharmacist Role
  • Established experience in functions beyond fundamental dispensing and order-processing activities such as optimization of medication regimen and monitoring or pharmacist-led care management
  • Demonstrated experience with population health and/or disease state management
  • Demonstrated ability to research and analyze medication literature including drug information, disease states, and clinical practice guidelines to identify gaps in care and create/communicate a recommendation plan to providers and patients
Preferred Qualifications
  • Board Certified Ambulatory Care Pharmacist, or Board-Certified Geriatric Pharmacist, or Board-Certified Pharmacotherapy Specialist
  • Pharmacy residency
  • Certified in MTM
  • Experience with Medicare STARS or managed care
  • Knowledge of clinical program components, formulary designs, prescription benefit trends
  • Previous leadership experience
  • Bilingual in English and Spanish

The typical pay range for this role is:$106,200.00 - $225,000.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
