Short Description
The Outpatient Social Worker is responsible for coordinating the care needs of patients and families suffering from Neurological disease and illness, as well as provide supportive counseling, psychosocial assessment and interventions for patients with complex social, medical, and financial needs.
The Neurology LCSW is expected to organize and maintain support group meeting monthly and will maintain the organizational relationship with The Parkinsons Foundation. The Social Worker also assists the interdisciplinary team to help the patient and families cope with hospitalization, illness, diagnoses, treatment, and other issues as they prevent a barrier to efficient patient throughput and quality outcomes.
Provides psychosocial assessments, crisis intervention, resources referrals, referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs as needed. Coordinates care authorization process with insurers if indicated.
Experience Required
Minimum of 1 year of clinical social work experience is required. Must demonstrate theoretical knowledge of psychology for Child/Adolescent, Adult/Geriatric populations.
Knowledge of principles and practices related to patients with neurological and movement disorder diagnosis and casework preferred but not required.
Strong critical thinking skills.
Bilingual in Spanish and English preferred but not required.
Education Requirements
Requires a master's degree in social work (MSW)/ or BSW or a licensed RN, BSN preferred.
License/Certification Requirements
Current NJ SW License, LCSW Preferred.
or
Current NJ RN License.
ACM (American Case Management Association) preferred.
CCMC (Commission for Case Manager Certification) preferred.
