Short Description

A nonexempt position responsible for the nursing care under the supervision of physicians, providers and/or registered nurse(s). May be involved in both the clinical and administrative areas including physical assessments, medication administration, promotion of wellness through patient education and teaching, care of the patient environment and organizational tasks. Maintains working knowledge of regulatory standards and is accountable to sustain these standards in daily operations. Initiates and implements patient teaching plans. Clinical direction, oversight and competency review is provided by medical providers and ambulatory professional development team in coordination with office management.

Cooper’s Urban Health Institute is looking for an LPN to join our multidisciplinary team. Our practice is dedicated to redesigning and improving care delivery to underserved populations, specifically Camden based Medicaid and Medicare beneficiaries and offers LPNs a unique opportunity to work directly with patients around chronic disease management in a collaborative practice with medical providers, psychologists, pharmacists  and health coaches to help improve health outcomes for patients facing challenging social determinants of health (homelessness, food insecurity, financial instability, transportation barriers, etc.)

 

The ideal candidate will have 13 years’ experience providing clinical services in an outpatient setting; bilingual English/Spanish preferred; excellent organizational and interpersonal skills, strong oral/written communication skills and an ability to work well with people of various backgrounds, ethnicities, and life experiences are required.   The successful candidate will have a desire to help improve health outcomes for patients, serve as a patient advocate as part of a multidisciplinary team and perform the following nursing tasks:   triage for patients to collect pertinent medical findings to report to the providers for plan of care development; work one on one with patients through LPN protocol visits for diabetes and hypertension under the direction of the Physician, Nurse Practitioner or Physician Assistant;  educate patients on appropriate self-management behaviors and skills for disease management and serve as a patient liaison between patients and external vendors (pharmacies, insurance companies, ME companies, homecare agencies, and medical day care facilities).  A successful candidate will find joy in doing something, no matter how small,  to improve the lives of the people we serve.

 


Education Requirements

Graduate of an accredited school of practical nursing.


License/Certification Requirements

Current New Jersey LPN License (Licensed Practical Nurse).

BLS issued by the American Heart Association
