Short Description

The Lead Nurse Navigator is an integral part of the Primary Care Multidisciplinary Team.

The Lead NN supports, educates, identifies barriers to care and assists patients and families with their primary care needs. This role helps patients navigate the complexity of the healthcare system and contributes to improved patient outcomes and patient and provider satisfaction. In addition, this position is responsible for the management and mentorship of Nurse Navigators working in Adult Primary Care.

 

Manages Nurse Navigator team caring for patients in Internal and Family Medicine

Leads Nurse Navigator team in the provision of quality, safe, efficient nursing care

Partners with Primary Care providers to provide optimal non-patient facing care via telephonic and/or electronic means

Utilizes telephone triage protocols to assess patients and determine a disposition for their care

Facilitates one call resolution of patient needs

Coordinates care for patient population

Delivers provider-approved medical advice to promote health maintenance and/or recovery from acute illness or minor injury

Partners with Amb Ops and Institute leaders in maintaining efficient processes in the provision of timely care

Complies with all system and departmental policies and procedures

Maintains active and current professional credentials


Experience Required

3-5 years of experience required


Education Requirements

Master's Degree Required


License/Certification Requirements

RN Degree
