Short Description

• Provide ongoing care-coordination and case management services. Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services to individuals with Substance Use Disorder.

• Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder

• Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome

• Provide continuity of care by meeting patients in various clinical care settings including the following: emergency department visits, addiction treatment appointments, medical appointments, social service appointments, and therapeutic appointments.

• Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc.

• Perform outreach by meeting with patients in the community and act as allies and advocates for patient services

• Attend Addiction Medicine clinical and programmatic conferences and meetings as required.

• Engage in ongoing professional development, training and receipt of clinical supervision.

Experience Required

1 year of relevant experience if candidate has Bachelor’s degree in related field or 2 years of relevant experience if candidate has Associates degree or is a Certified Peer Recovery Specialist.

 

4 years of relevant experience, either clinical or lived, if candidate is a Certified Medical Assistant.

 

OR 4 years of relevant/lived experience


Education Requirements

Bachelors’ degree in a related field, plus 1 year of relevant experience working with patients with Substance Use Disorder 

OR Associates degree with 2 years of relevant experience

OR Certified Peer Recovery Specialist designation plus 2 years relevant experience.

OR Certified Medical Assistant with 4 years of relevant experience.


License/Certification Requirements

Valid driver’s license.  Must possess own vehicle or reliable alternate means of transportation for use to outreach individuals served.
