Summary:
Utilizing a collaborative process with the Complex Care Coordination (CACO) Social Worker, the RN Case Manager will assess, develop, and implement a plan of care to meet the individual health care needs of children with complex medical problems or special health care needs, who have multiple specialists and who are at risk for frequent healthcare utilization Using effective communication skills and available resources, the RN Case Manager will provide support and reinforce education across multiple service delivery settings in an effort to meet patients unique needs and access care for quality, cost-effective outcomes. The RN Case Manager in conjunction with the CACO Social Worker: Maintains an onsite presence to do timely real time reviews of patient referrals. Exceptions to this expectation will require Director approval. Assists in coordinating effective use of hospital and community resources. Facilitates communications among patient, physicians, family, and the patients primary care physician (PCP) by phone, e-mail, text, or in person.

• ffers supportive services including listening, counseling (developmental), and reinforcement of education, Advocates for the needs of family/patient. Attends case conferences and/or meetings on or off site as required. The RN Case Manager will work within a multidisciplinary team to ensure patients receive quality and compassionate care tailored to their individual needs.
Education and Experience
- Associate degree in nursing and at least three years of any combination of experiences working in/with case management, care coordination, utilization review, patient intake, discharge planning and troubleshooting fund resources, quality assurance, clinical pathways, continuous quality improvement, or state and federal health plans or commercial insurance plans in a clinical or managed care environment.
- Must be computer literate, and have effective organizational, interpersonal, written, and oral communication skills.
- Must be familiar with various community resources and charitable organizations.
- Must be able to adapt to changing healthcare environments and work with all members of the healthcare team to achieve positive outcomes.
- Must have experience using or navigating electronic medical records (e.g., EPIC, Meditech, Healthy Planet, etc.).
- BSN from an accredited college or university is preferred.
- Bilingualism is preferred or may be required depending on the specific department assignment.
Licensure, Registration, and/or Certification
- Current RN license from Texas Board of Nursing required.
- Must achieve a Basic Life Support (BLS) certification for Health Care Providers within 30 days of employment and must maintain this certification as per Medical Center Policy, MC 175.
This requirement does not apply to Health Plan
- Case Manager Certification (CCM) preferred.
