Summary:
The Manager of Patient and Family Support Services functions in an expanded healthcare role within the Cook Children's Neighborhood Clinics to collaborate with the Primary Care Provider, members of the patients healthcare team and patients/families to ensure that the patients psychosocial needs are addressed appropriately. This position directly performs social work activities for patients, as well as, supervises case management and community engagement efforts performed by family advocates. The manager will act as process owner and subject matter expert for all assigned patient and family support programs to enrich the patient experience within the Neighborhood Clinics. This individual will work with many other CCHCS departments to implement and manage population health initiatives that support CCHCS strategic goals.

Education and Experience:
- Masters level social work degree and licensure required (LMSW) or (LCSW)
- Five years of social work experience including working with children and families
- Two years of experience working in a healthcare setting
- Minimum 1 year of supervisory experience to include managing the performance of others
- Bilingual English and Spanish a plus

- Ability to plan and organize effectively
- Proficient in use of computer applications
Certification/Licensure:
- Masters level Social Work license (LMSW or LCSW) current, unrestricted Texas license
