Summary:

The Behavioral Health Care Coordinator provides care management services to members with low to moderate health complexities. Activities include, but are not limited to outreach, assessment, service coordination, care management, care service planning, and member support.

Additional Information:

Education:

  • Requires a BS/BA of Social Work/Psychology/Mental Health or related field. Community Health Worker (CHW) training and certification will be required within 12 months of hire.
  • Minimum of one to three (1-3) years experience in community health, social services, community advocacy, community outreach, or a combination of relevant experience and education.
  • Bilingual candidates preferred but not required.
  • Understanding of the community in the Service Delivery Area through shared experiences and/or a strong desire to help people in vulnerable communities.

  • Strong skills in the following areas: Experience working with children, adolescents and adults who are diagnosed with behavioral health disorders and/or substance use disorders.
  • • ral and written communication; Critical thinking; Organization and time management; Proficiency in Microsoft Office and internet/web navigation.
  • Private reliable transportation
  • Proficiency in navigating Electronic Medical Record
  • Customer service.
  • Ability to establish and maintain positive and effective work relationships with coworkers, members and providers& remotely upon orientation completion
  • Bi-lingual (English/Spanish) Preferred. No testing requirement

Certification/Licensure:

  • Texas Community Health Worker Certification.
  • Valid Texas Driver's License and proof of insurance
