Additional Information:
Delivers a variety of activities for the health plan and local community members seeking services at a CCHP community education site. These services are designed to improve individual health outcomes and provide essential support to members referred by onsite clinical and community service teams. These activities may involve facilitating access to Telehealth, organizing educational classes, coordinating food distribution, providing member support, and assisting with clinical services.

Education & Experience
- High school diploma or GED.
- 3 years' experience in community health, social services, community advocacy, community outreach, health plan member services, or a combination of relevant experience and education.
Knowledge, Skills & Abilities
- Must have an understanding of community in the Service Delivery Area including an ability to establish and maintain positive and effective work relationships both internally and externally.
- Must have the ability to communicate with written and verbal proficiency in English and Spanish.
- Must have a basic knowledge of medical terminology.
- Must be proficient in and able to demonstrate computer skills include experience using Excel, Word, Microsoft Office Suite products (e.g. Word, Excel, Outlook, Power Point, etc.).
Licensure, Registration, and/or Certification
- Community Health Worker (CHW) training and certification required.
