  • Gestion de la base de données et vérification continue des dossiers et de l'information saisie;

  • Assurer la liaison avec les fournisseurs de soins de santé, les pharmacies et les cliniques de perfusion et d'autres organismes au besoin, afin de coordonner les perfusions, l'administration de médicaments et résoudre les problèmes qui surviennent;
  • Entretenir des communications claires avec le gestionnaire, à l'équipe et le client concernant la gestion des cas de patients;
  • Avec les gestionnaires, participer au processus de formation de nouvelles infirmières gestionnaires de cas, y compris, mais sans s'y limiter, l'observation et la surveillance des appels;
  • Soutenir et collaborer avec le personnel d'autres programmes en vue de l'élaboration et de la mise en œuvre de programmes de gestion des maladies, incluant l'établissement des exigences cliniques, la rédaction de textes, de manuels ou de dépliants pour les patients, la détermination des paramètres du programme de conformité, des évaluations des besoins et des protocoles;
  • Fournir des commentaires au sujet des protocoles, bases de données, processus et relations entre départements actuels
  • Être disponible sur appel en dehors des heures régulières et le week-end (24/7) pour des consultations téléphoniques périodiques et des courriels en fonction des besoins du programme;
  • Se charger de signaler les effets indésirables aux unités/départements qui lui sont confiées;

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Provide support to patients and health care providers, including but not limited to information on the Patient Support services, specific diseases and therapies based on manufacturer provided materials and product monograph;
  • Work with patients and families to complete a needs assesment related to medication access;
  • Acts as a liaison between patients, families, health care providers and payers related to medication access.
  • Provide answers to general inquiry calls submitted via telephone or web-based platforms on program services, disease state or product related questions, based on information provided as part of the program materials
  • Provide ongoing support on disease state or product related questions, based on information provided as part of the program materials to ensure patient adherence, as dictated by the needs of the patient.
  • Refer the patient to contact their healthcare provider should the patient have clinical questions, assessments and situations that fall outside program provided materials or manufacturer product monograph;
  • Database management and ongoing auditing of files and information captured;
  • Administer and adhere to case-management protocols, including data collection, on-going patient follow-up, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers;
  • Work directly with the reimbursement specialist and the physician to secure funding for their patient's therapies; assist with alternative funding searches when needed;
  • Prepare for and attend extensive, specialized training sessions for specific therapies provided by the pharmaceutical team, as required;
  • Attend pharmaceutical meetings as required;
  • Attend patient meetings, conferences as needed to educate patients and providers on Patient Support Program services;
  • Work with patient associations and advocacy groups, as required;
  • Establish and maintain partnerships with pharmaceutical team;
  • Liaise with health care providers, pharmacies and infusion clinics, and other agencies as required, to coordinate infusions, drug delivery and resolve issues as they arise;
  • Clear communication to the manager, team and the client regarding the management of patient cases;
  • With the managers, participate in the training process of new Drug Access Navigator, including but not limited to shadowing & call monitoring.
  • Provide feedback regarding current protocols, databases, processes, inter-departmental relationships
  • Report Adverse Events / Severe Adverse Events following program Standard Operating Procedures;
  • The Drug Access Navigator will also be assigned other duties and tasks as required from time to time. May be required to be on standby/ on call as part of this role.

What your background should look like

EXPÉRIENCE ET NIVEAU D'ÉDUCATION REQUIS :

  • Baccalauréat ou diplôme d'infirmier(ère) autorisé(e) (IA), diplôme d'études supérieures ou expérience spécifique de gestion d'un cabinet est préférable, avec cinq (5) années d'expérience en soins infirmiers;
  • Au moins deux (2) ans d'expérience du contexte du remboursement par des tiers publics ou privés ou dans les ventes (non requis, mais un atout), la gestion des soins ou le soutien clinique dans l'industrie pharmaceutique, ou expérience connexe;
  • Le(la) candidat(e) préféré(e) est un(e) infirmier(ère) avec une expérience dans les soins de santé et la gestion de cas, dans un cabinet médical ou un hôpital ou dans l'industrie de l'assurance ou de la biopharmaceutique;
  • Bilinguisme (français et anglais) est préférable;
  • Expérience en hématologie et/ou néphrologie constitue un atout.

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

  • A Bachelor's degree inhealth care or a related field
  • Requires a minimum of three (3) years, and preference of greater than five (5) years, in the healthcare, pharmaceutical or insurance industry or related experience;
  • The preferred candidate has healthcare and case management experience in medical or hospital practice or the insurance or bio-pharma industry;
  • Bilingualism (French and English) strongly preferred;
