As a VAD/ECMO Navigator, you will be responsible for the coordination and management of care of the patient, from referral for MCS throughout their lifetime while on durable MCS support! This is a salaried, Monday-Friday, Day shift position, which does include 24-hour on-call rotations 7 days/week. Specific working hours vary depending on patient and program needs.

POSITION SUMMARY
This position provides comprehensive management and coordination of inpatient and outpatient care for specialized patient populations, such as those requiring ventricular assist device or artificial heart. This position also has accountability for coordination of activities including, but not limited to, patient education, staff education, community education, surgical coordination, and record keeping.

CORE FUNCTIONS
1. Manages individual patients across the healthcare continuum to achieve optimal clinical, financial, operation and satisfaction outcomes. Facilitates, coordination, and monitors pre-implant evaluations and the cares of implant candidate waiting implantation. Collaborates effectively with the interdisciplinary team to plan and implement the patient's treatment plan, utilizing effective oral and written communication. Participates in weekly selection and patient update conference.

2. Collects required clinical data and collaborates with case manager of insurance provider for approval. Interprets and administers current payor contracts to best meet the financial needs for the patient and program while maximizing facility and program reimbursement. Possesses integral knowledge of multiple patient contracts. Applies this knowledge to assure the services being furnished to the patients are in compliance with the contract stipulations, obtains appropriate implant authorizations for services rendered. May collaborate with finance representatives regarding insurance coverage.

3. Establishes and promotes collaborative relationships with physicians, members of the health care team and/or third party payors, utilizing effective oral and written communication. Serves as a resource to, health care providers, patients, physicians and their office staff and/or payors. Educates other health care providers on individual care policies as it pertains to service rendered. May assist with preparation and start up activities in operating room as needed by surgical team.

4. Provides post-operative and/or procedural education to patients and family in collaboration with nursing staff in preparation for discharge from the hospital. Provides support and education to nursing staff and other health professionals regarding the device or procedure. Develops and presents education and marketing programs.

5. Participates in the program quality assurance program. Evaluates current processes, identifies need for change and implements change via process improvement projects to achieve optimal clinical, financial, operational and satisfaction outcomes. Acts as a change agent by continually monitoring processes to identify opportunities for improvement. Implements change through multi-disciplinary work group process. Assists in development of policies and procedures for the delivery of care to the patient population. May be responsible for maintaining/assisting with related database, software and programs.

6. May provide support to patients leaving the unit for tests and procedures. Serves as a resource to the community, patients, family members, primary care physician and payors regarding the implant process. Provides support and education to community health care professionals who have contact with the patient i.e. home health nurses, referring physicians, first responders (EMTs, paramedics, and local emergency departments). Serves as resource person to patients after discharge for clinical and technical support (after hours for patient call responsibilities.) Coordinate readmissions for patients experiencing difficulties.

7. Internal customers include patients, families, in-plant surgeons, in-plant interdisciplinary team, management, nursing and medical staff, and other members of the hospital interdisciplinary health care team. External customers include referring physicians and office staff, laboratories, third party payers, pharmacies, community agencies, provider networks, and regulatory agencies.

MINIMUM QUALIFICATIONS

Knowledge and skills as typically acquired through a bachelor's degree in a biological science such as biology, chemistry, anatomy, nursing, etc. is required Clinical knowledge as typically obtained through an RN or equivalent health care program. Requires a minimum of five years clinical experience; at least two of those years in the clinical specialty area. Must have a working knowledge of care management, resource/utilization management, and hospital and community resources. Must demonstrate clinical proficiency in the care of the specialized patient population. Requires critical thinking skills to effectively coordinate a large volume of patients simultaneously. Must demonstrate problem-solving abilities, effective oral and written communication skills, and time management skills. Must demonstrate ability to work effectively in an interdisciplinary team format.

PREFERRED QUALIFICATIONS

Master's degree in related area and/or certification in area of specialty preferred.

Additional related education and/or experience preferred.
